Regional Medical Center Bayonet Point has opened the Outpatient Heart Failure clinic to better serve our patients. This clinic was designed to offer a comprehensive disease management program for patients diagnosed with heart failure. Currently approximately 5.1 million people in the United States have been diagnosed with heart failure, with 550,000 new cases diagnosed each year. Regional Medical Center Bayonet Point recognized a community based need for special care of the heart failure patient. This new clinic located on the grounds of Regional Medical Center Bayonet Point, offers heart failure patients the opportunity for urgent evaluation and treatment, customized patient and family education, and guideline directed medical therapy. The clinic utilizes laboratory studies, EKG monitoring, and radiologic studies to evaluate and monitor the effectiveness of therapies. Our clinic is overseen by a Board Certified Advanced Registered Nurse Practitioner who acts as an extension of our local healthcare providers.
The Outpatient Heart Failure Clinic at Regional Medical Center Bayonet Point assists in the immediate care provided to heart failure patients in the local community. Staffed by an Adult Health Board Certified Nurse Practitioner, the clinic acts as an extension of our local healthcare providers.
The goal of Regional Medical Center Bayonet Point's Outpatient Heart Failure Clinic is to bridge the gap between the patient's hospitalization and outpatient follow-up with the patient's cardiologist and primary care physician. By bridging this gap the patient is re-evaluated quickly after discharge and receives any additional education or treatment required, including IV diuretic therapy, to help prevent hospital readmission.
